Notice to Veterans: The VA Health Information Exchange Opt-Out Form, VA Form 10-10164, is now available online at, on this website, or in Release of Information Office (ROI) at VA medical centers.  The form must be signed, mailed and/or hand-carried to a Veteran's local Release of Information Office.  Currently, there are no means of submitting the form electronically.   VA Systems will begin opting all Veterans into health information sharing, beginning January 2020.*** So again going back to the mail or drop off the form to your closest Release of Information Office. No urgent deadline but for those who want to opt-out send it in now while it is on your mind. Info regarding this program (ignore the Tricare mention of Templates). Veterans Health Information Exchange (VHIE) Home ;                *** Note: the third page of the letter mentions releasing information regarding debts to credit unions. This is done by government agencies and civilian businesses to report delinquent debts. This will not change. We aren't able to opt-out of that.**
